At Kingswood Medical Group we recognise the role of a Carer and are committed to providing support to those individuals who are recognised as Carers for our patients and those patients registered at other Surgeries. The Carer Registration Form can be found below, and should be completed as soon as possible.
Once the Registration Form is received a note will be made on the medical records of the Carer and where appropriate the records of the individual(s) they care for. This will allow us to be more aware of the challenges that can be faced with regard to time constraints etc when dealing with Carers.
We hold regular Coffee/Tea Sessions at the Surgery for Carers, notices are displayed and personalised letters are sent to advise Carers of the sessions. At these sessions members of the Surgery are available to talk to the Carers as are other agencies that provide help and support.
We work closely with Swindon Carers Centre and have a nominated member of our reception Team who is available to help – Jan is a carer herself so she understands the problems that can arise.
